What should I do if...

  What should I do if I get married?
  If you get married, simply print an enrollment form.  Fill out the form in order to add your spouse to the health plan.  Your spouse is covered as of the day of your marriage.  Mail the form to the Trust office.  You may also want to fill out a new beneficiary form, fill it out, and include and mail it to the Trust office also.If you so chose, you also have the opportunity to change health plans at this time.

  What should I do if I get divorced?
 

If you become legally separated or divorced, your are responsible for contacting the Trust office to let us know the date of separation or divorce.  Let us know the name, social security number, and address of your spouse so that we may send him/her a COBRA notice if they wish to continue health coverage. 

You may also want to fill out a new beneficiary form to change the beneficiary for your life insurance and pension.  Simply fill out the form, have it signed by a witness (who is not a beneficiary) and mail it to the Trust office.

If you so chose, you also have the opportunity to change health plans at this time.

  What should I do if I move?
  Simply email or mail us your new address so we may change our records.  If you move into or out of an HMO or PPO service area you may have to change health plans or have the opportunity to change health plans.

  What should I do if I have a child or adopt?
  If you add a member to your family, simply print an enrollment form.  Fill out the form in order to add your child to the health plan.  Your child is covered as of the day of your birth or adoption.  Mail the form to the Trust office.  You may also want to fill out a new beneficiary form, and include and mail it to the Trust office also. If you so chose, you also have the opportunity to change health plans at this time.

  What should I do if I get legal custody of a child?
  If you add a member to your family, simply print an enrollment form.  Fill out the form in order to add your child to the health plan.  Your child is covered as of the day of custody.  Mail the form and a copy of the court document granting custody to the Trust office.  You may also want to change your beneficiary form, and mail it to the Trust office also. If you so choose, you also have the opportunity to change health plans at this time.

  What should I do if there is a death in my immediate family (or I die)?
  Contact us at A&I in order to find out how the survivors' benefits may be affected, and the options the survivors' have.  Depending on the circumstances a photocopy of the death certificate may be required.
